Real-Time Social Determinants of Health Solutions Now Available from LexisNexis 

API delivery can help healthcare organizations improve health outcomes based on clinically validated social determinants of health

ATLANTA — In March, LexisNexis® Risk Solutions announced the release of its new Socioeconomic Health Score that can help predict hospital readmission risk. Now, with the new release of easy-to-integrate APIs, healthcare organizations can have access to real-time delivery of the Readmissions Risk Score, one of several scores available from the LexisNexis Socioeconomic Health Score suite. The clinically validated attributes and risk score are available to all types of healthcare organizations – providers, payers, pharmacy and life sciences.  

With real-time socioeconomic information, the cutting-edge data analytics model can help caregivers make immediate care decisions, including helping to assess the likelihood of a patient being readmitted to the hospital within 30 days. 

“Seeing health alerts as care givers are interacting with a patient can help the care team initiate meaningful conversations about how to address a patient’s social determinants,” said Erin Benson, director, Market Planning, LexisNexis Risk Solutions. “That could include someone doing discharge planning having access to the readmission risk score while the patient is still in the hospital or a pharmacist providing a consultation at the counter.”

Social determinants of health affect overall health outcomes more than medical determinants, according to County Health Rankings. Research conducted by LexisNexis indicated that the Readmission Risk Score is on par with—or better at predicting risk than—readmission scoring models that use only clinical data, which is often difficult to obtain. 

To compete in today’s healthcare environment, it is vital that healthcare providers improve risk stratification and adapt care management strategies to personalize care for the most at-risk patients. Having real-time access to risk scores that can help enable on-the-spot care decisions is a significant step in improving the healthcare process.
